We offer health checks for patients with ASTHMA, COPD, DIABETES, HYPERTENSION, HEART PROBLEMS and other chronic conditions and we advise all patients with these conditions to be seen annually for a full check.
Patients over the age of 75 who do not have annual checks for chronic conditions and have not had a check up in the previous 12 months are welcome to request a routine health check.
Patients between the ages of 16 and 74 who have not had either a consultation or health check in the previous3 years may request a routine check.
All patients on repeat medication need to attend for medication reviews.
Childhood Immunisations - If your child is due to attend for immunisations you should receive an appointment card from Coventry Primary Care Trust. This will schedule you into our clinic, which is held on Tuesday mornings.
It is important that you attend at the arranged time. If this is not convenient, we are happy to arrange an alternative time. If, for some reason, you do not receive an appointment please phone the surgery and we can arrange this for you .
Child Health Clinics - These are held by our health visitor by appointment, if you are unable to attend or, if you would prefer to attend the local clinic, please phone and let us know, this enables someone else to be seen . It is very important that you attend at the arranged time.
Ante-Natal Clinic - This is organised and held by the midwife and an appointment is essential – please ask at reception .
Teenage Concern - Teenagers aged 14 or over are welcome to discuss any problems or concerns – our practice nurse may be able to help or advise. All patients should have booster Tetanus & Polio immunisations at school leaving age.
Minor Surgery - There is often a waiting list for this and we will contact you either by phone or letter when an appointment is available – it is important that you let us know as soon as possible if you cannot attend.
